At the conclusion of each membership year, 4-H members in Cooke County are recognized for all of their hard work and accomplishments. It is a time to not only award 4-Hers for their work, but to also recognize the volunteers and adult leaders involved in the Cooke County 4-H Program.

An invitation to the annual banquet is earned when members have met certain requirements of participation in projects and ribbon activities.

PROJECT RECORD FORMS

4-H members are required to submit a project record form for each of the projects they participated in over the past year (since August of 2021). Please refer to the Texas 4-H Project List for details of what constitutes a project:  Texas 4-H Project List

Deadline to submit forms is June 30, 2022. pdf and word formats have been provided. Project record forms can be accessed below by selecting the link:

RIBBON ACTIVITY FORM 2022

4-H members are requires to submit a ribbon activity form for each activity they participated in over the past year (since August of 2021). Deadline to submit forms is June 30 2022. The ribbon activity form can be accessed here:

SPECIAL AWARD APPLICATIONS FOR BANQUET

There are also special awards  presented at the banquet to outstanding 4-H members. The Final deadline  to submit special award applications is  5:00 pm, July 15, 2022.

Links to applications for all awards are listed below and categorized by 4-H age. Select the award you qualify for and are interested in applying for, fill in the information, obtain the appropriate signatures and turn COMPLETED FORM into the Extension office by the deadline.  NOTE: INCOMPLETE FORMS WILL NOT BE ACCEPTED.

Some awards  require an interview, which will be stated on the application. The date for interviews will be Friday, July 22, 2022 beginning at 3:30 pm. Applicants must schedule an interview at the time their application is submitted.

SCHOLARSHIP APPLICATION FOR BANQUET

Cooke County 4-H Scholarships are academic scholarships awarded each year to applicants based on their participation in 4-H work, leadership/character and scholastic ability. Applicants must be entering their Senior year this fall and graduating in May 2023 from a private, public or home school and have been a Cooke County 4-H member for at least 5 years.

All applications must be submitted by Friday, July 15, 2022 at 5:00 pm

All applicants must be present for an interview before the scholarship selection committee on Friday, July 22, 2022.

Interview times will be scheduled upon submission of application.

Please review the guidelines and rules outlined in the application to ensure eligibility and proper completion of application.
